Privacy and Confidentiality
At Port Recovery IOP, Inc. and Port Recovery Mental Health, Inc., protecting the privacy and confidentiality of our patients is very important to us.
Your Health Information
During the course of providing services, we may collect and maintain information about you and your care. This is commonly referred to as Protected Health Information (PHI) and may include information such as your name and contact information, health history, diagnoses, treatment records, medications, insurance information, and other information related to the services you receive.
We take appropriate steps to protect this information and keep it confidential.
How We Use Your Information
Your health information may be used as necessary to provide and coordinate your care, operate our programs, and obtain payment for services. This may include communication among members of your treatment team and, when appropriate, communication with health insurance companies or other organizations involved in your care or payment for services.
We only use or share health information as necessary and as permitted or required by applicable privacy and confidentiality requirements.
Sharing Your Information
We do not disclose your confidential health information to outside individuals or organizations simply because they request it.
When your permission is required, we will ask you to complete a written Authorization to Release Information identifying what information may be shared and with whom. You may also ask us to communicate with another health care provider, family member, attorney, agency, or other person involved in your care when permitted by applicable confidentiality requirements.
There are limited circumstances in which we may be required or permitted to disclose information without your authorization, such as a medical emergency, suspected abuse or neglect, a serious threat to health or safety, certain legal requirements, or authorized oversight and regulatory activities.
Substance Use Disorder Information
Information related to substance use disorder treatment receives particularly strong confidentiality protection. We take additional precautions when using or sharing this information and obtain your authorization whenever it is required.
Your Privacy Rights
You have rights regarding your health information. Depending on the circumstances, these may include the right to review or obtain a copy of your records, request a correction, request certain restrictions on how your information is used or shared, and ask us to communicate with you in a particular way.
You may also ask questions about how your information is protected or raise a concern about the privacy of your information. We will not retaliate against anyone for asking a privacy question or raising a good-faith privacy concern.
